    How Stress Undermines Brain Circulation and May Raise Dementia Risk

    You know that drained, foggy feeling that shows up on stressful days? That isn’t random.

    Stress disrupts the system that delivers blood and oxygen to your brain, and once that balance slips, everything from memory to clear thinking starts to suffer. The real problem is how it happens long before you notice anything slipping.

    The brain depends on

    precise coordination between its electrical activity and its blood supply. When neurons fire, blood vessels must quickly deliver oxygen and nutrients to support that activity.

    This moment-to-moment matching

    (called neurovascular coupling) is essential for attention, memory, and clear thinking(1).

    A key player in this system is nitric oxide, a molecule released by certain neurons that signals nearby blood vessels to widen(2). Think of it as a rapid communication line between active brain cells and the vascular system, ensuring that areas working hardest receive

    the fuel they need.

    Two Types of nNOS Neurons

    Researchers generally describe nNOS-expressing neurons in two categories:

    • Type-I nNOS neurons:

      These cells are rare but powerful. They produce large amounts of nitric oxide, send both local and long-range signals, and reliably trigger strong increases in blood flow when activated(4).

    • Type-II nNOS neurons:

      These are far more common but produce much less nitric oxide and have a more modest impact on circulation(5).

    Among these nitric-oxide-producing neurons, type-I nNOS neurons stand out.

    Although they represent only a tiny fraction of cells in the cortex, they release high concentrations of nitric oxide and have an outsized influence on blood flow. They are activated during sensory experiences,

    voluntary movement,  and other states that require the brain to boost circulation(3).

    Type-I cells also have a distinctive receptor, TACR1 (NK1R), which allows them to respond to substance P, a chemical released during intense network activity. When substance P activates them, blood vessels remain dilated for an extended period, supporting sustained activity in the surrounding tissue(6).

    Why These Neurons Matter

    Despite their small numbers, type-I nNOS neurons appear to coordinate activity across the brain. They help keep the left and right hemispheres synchronized and contribute to

    slow brain rhythms that support restorative sleep. These rhythms also help circulate cerebrospinal fluid, which clears metabolic waste from brain tissue.

    What The New Study Tested

    To understand what happens when these neurons are lost, researchers used a targeted toxin to remove only type-I nNOS neurons from the somatosensory cortex. The rest of the surrounding circuit was left intact(3). This allowed the team to observe how the system changes when a key regulatory cell population is missing.

    What Happened When Type-I nNOS Neurons Were Removed

    The effects were broad, despite the small number of neurons involved:

    • Weakened blood-flow responses during prolonged stimulation: Blood vessels still responded, but the increases were significantly reduced.
    • Loss of a normal ‘after-response’: In healthy brains, blood vessels briefly constrict after neural stimulation ends, which is a sign the system is regulating properly. In this study, that pattern disappeared.
    • Reduced low-frequency brain rhythms: Electrical activity in the 1–4 Hz range, which supports deep rest and waste clearance, dropped noticeably.
    • Decreased coordination between hemispheres:

      Neural activity and blood-flow patterns became less synchronized across the two sides of the brain.

    • Dampened resting blood-vessel pulsations:

      These natural pulses help propel cerebrospinal fluid, while weakening them may impair waste removal.

    Together, the findings suggest that type-I nNOS neurons function as quiet but crucial organizers, helping align electrical activity with vascular responses across the cortex.

    Rethinking Their Role

    Earlier work showed that stimulating these neurons produced strong changes in blood flow but relatively modest changes in overall neural activity. Removing them, however, revealed something different: they appear essential for maintaining stable rhythms, interhemispheric coordination, and

    healthy vascular dynamics.

    This pattern suggests their role is broader than simply widening vessels. They help shape the brain’s underlying structure of communication.

    Why This Matters for Brain Health

    Type-I nNOS neurons are unusually vulnerable to stress, inflammation, and aging.

    Their gradual loss may contribute to:

    • Impaired regulation of blood flow
    • Reduced brain-waste clearance
    • Disruptions in coordinated brain rhythms
    • Increased susceptibility to neurodegenerative conditions

    When these cells fail, the brain’s communication and circulation systems drift out of alignment. Over time, that instability may raise the risk for cognitive decline.

    Protect Your Brain Health by Protecting Its Most Vulnerable Cells

    Because type-I nNOS neurons are so sensitive to chronic stress, systemic inflammation, and

    age-related biological strain, their decline may represent an early tipping point in the pathway toward cognitive vulnerability. These cells help keep brain activity, blood flow, and waste-clearance systems aligned. When they falter, the entire neurovascular network becomes less resilient.

    This makes stress management, metabolic health, and inflammation control especially important as we age.

    Practices that reduce physiological stress such as consistent sleep, regular physical activity, mindfulness-based stress reduction, and balanced nutrition may help preserve the function of these neurons and the vascular signals they provide. Supporting cardiovascular health, limiting chronic inflammation, and avoiding long-term elevations in stress hormones may also help maintain the brain’s ability to match blood flow to neural demand.

    long-term cognitive resilience.
